You can speed up the check-in process in our office by completing online paperwork before your child's first visit. In order to decrease the amount of duplicate information to fill out for each child we need only one New Patient Packet completed for each family. For each additional child, you need only complete the forms found in the Additional Child Form in the box below. Please either print these documents upon completion and bring to your first appointment by clicking PRINT or submit to them via email by clicking SUBMIT. You will receive a personalized email confirmation within 48 hours from Eryn at our office once the documents are received electronically.
If you wish us to file dental insurance claims on your behalf, please bring your insurance card to your child's appointment for verification purposes. Upon check–in for your appointment, Eryn, our receptionist, will ask you to sign a paper record of your electronic forms since electronic signatures are not yet available in our office.
